The application forms can be found in the “Download” section on the main page. These should be filled in and signed by a consenting adult. The completed application form, 2 passport size pictures and the signed CLS form, should be sent in with a cheque for the corresponding amount. The registration fee and the final amount must be sent in before the deadline.
